CERTIFICATE OF DISABILITY FOR DISCHARGE

Andrew J. Flory, a Private of Captain Frank Leepers Company A of the Eighth Regiment of United States Volunteer, was enlisted by G. M. Bruce of the 8th Regiment of Ills Infty at Decatur, Macon Co, Ills on the 25th day of July 1864, to serve 3 years; he was born in Long Creek in the State of Illinois, is 21 years of age, 5 feet 7 inches high, Dark complexion, Black eyes, Dark hair, and by occupation when enlisted a Cooper. During the last 5 months said soldier has been unfit for duty.
Said A.J. Flory was wounded by a gun shot in the face at Fort Donoldson Feby 15/62 and has not been able to perform Military duty since.

I certify that I have examined the said Andrew J. Florey of Captain Leeper's Company and find him incapable of performing the duties of a soldier because of a musket ball passing through the face fracturing very badly the lower jaw in two places and very much disfiguring and crippling the action of the jaw and mouth. Degree of Disability, Total.

ACT OF MAY 11, 1912
DECLARATION FOR PENSION.


State of Oregon, County of Jackson
On the 23rd day of May, A.D. one thousand and nine hundred and twelve personally appeared before me a Norary Public within and for the county and State aforesaid, Andrew J. Florey who, being duly sworn according to law, declares that he is 72 years of age and a resident of Eagle Point, county of Jackson, State of Oregon; and that he is the identical person who was enlisted at Cario, Ills, under the name of Andrew J. Florey on the 25th day of July, 1861 as a Private in Company, A, 8th Ills Inft in the service of the United States, in the Civil War, and was honorable dischared at Pittsburg Landing Tenn., on the 15th day of August, 1862. That he also served in Company A 8th Ills Inft from February 24th 1863 to February 23rd 1866.

That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 7 1/2 inches; complexion, fair; color of eyes, dark; color of hair, dark; that his occupation was farmer, that he was born April 20th, 1840, at near Decatur, Macon Co, Ills

That his several places of residence since leaving the service have been as follows: Decatur Ills to Oct 1867, Jacksonville to 1869, Salina, Kans to 1871, Floreyville, Kans to 1878 & Delta, Colo. 1882 & Eagle Point from 1882 to 1912.

That he is a pensioner under certificate No. 27232. That he has applied for pension under original No. _______

That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912. (Being totaly disabled for manual labor)

That his post-office address is Eagle Point, county of Jackson, State of Oregon
